Aged Care Navigators Brisbane
Getting Started
New to My Aged Care or stuck in the process? We’ll guide you through the complex aged care process – from accessing My Aged Care through to the assessment outcome, so you or your loved one will achieve an appropriate outcome.
There’s no place like home.
For many older people, home is a place where they feel safe, independent and in control.
So whether it’s a helping hand with the cleaning, shopping for groceries, or a lift to a social outing – it’s possible to get the support at home you need to keep living a life you love... a life that’s yours.
We’re aged care navigators – helping you move through the process and get the right outcomes for your needs.
Why a ‘navigator’?
Because the aged care system in Australia is complex and overwhelming.
How do you know what choices are out there?
Who will listen to your needs?
And how can you get the best in-home care for peace of mind?
Joyly exists to simplify aged care – to see more older Australians age with choice. We’re the compassionate, expert and hands-on support that gives peace of mind to family and friends – and independence and joy to older people.
Through My Aged Care, there are two main ways you can ask for help at home. The Commonwealth Home Support Programme (CHSP) for low-level support, and the Home Care Packages (HCP) for more complex care needs.
At Joyly, we understand that some older people starting with My Aged Care will already have complex care needs and need to move through the steps as quickly as possible.
That’s what we’re here for.
Who is the Getting Started package for?
Older people that are new to My Aged Care

How does it work?
-
Step 1 - Get to know you
An in-person / online visit to you or your loved ones home. We’ll perform a full clinical assessment – including medical, psychological, physical, social and complex needs – so we can truly understand YOU, apply for an assessment through My Aged Care, and start planning.
-
Step 2 - Explain the process
By working through the process with you – what you want and need – we will simplify the steps of a complex and confusing assessment system.
-
Step 3 - Assessment
Once you’ve heard from My Aged Care, we’ll attend the Home Support or Comprehensive Assessment at your home (occasionally it’s over the phone – but we’ll still be there). We’ll arrive early to prepare you for what’s to come and advocate for you at every step of the way.
-
Step 4 - Access services
We’ll use our industry knowledge to guide you to implement low level CHSP services into your home.
If you're new to My Aged Care, Getting Started packages start from $3,000.
Navigate the aged care system with hands-on support from Kym – a registered nurse with specialised knowledge in complex conditions.
Joy aside, there are a few things that make us different to many aged care navigators. For starters, our in-person process is led by Kym, a people person, advocate for YOU, and firm believer that older people can choose to live at home for as long as possible.
What’s more, Kym is a registered nurse with a deep understanding of the aged care landscape and a background in managing complex health conditions. What all this means for you: you have an ally on your side to help you live life on your terms.
"Excellent and professional service. Kym went the extra mile to establish what we as a family wanted and took it from there! Assessment, visits, forms… it was all delivered in a professional and friendly way."
